If
under
the
age
of
18,
the
form
requires
the
signature
of a
parent,
guardian
or
carer.
Send

by
post

if

is
urgent,
it
can
be
faxed
to

by
first

TO
SEA
STAFF
If
the
personal
details
required
on
the
form
have
not
changed
since
you
last
sailed
with
us,
there
is
no
requirement
to
complete
another
form.
However,
if
you
are
sailing
with
us
as a
volunteer
member
of
sea
staff
for
the
first
time
or
if
you
are
on a
familiarisation
voyage,
a
form
must
be
completed
and
sent
to
us
one
week
prior
to
embarkation.
